Preceded in death by paternal Grandfather Melvin Putnam (1997) and maternal Aunt Elizabeth Murray in infancy (1988).

The family would like to thank Midwife Lisa Chappell, Dr. Christine Dellaire and Dr. Karolyn Hardy-Brown as well as the care teams at Peterborough Regional Health Centre, Kawartha Midwives, Sick Kids and Mount Sinai for their remarkable dedication, kindness and compassion in caring for Jack, Janelle and Brad.
